Here you'll find my consent form and other relavent documents that we will need for the program. Please click and print what you need as we need them.

All of the forms listed below are provided in a PDF format for ease of printing. If you have issues printing any of these documents, please let me know and I'll provide you with a copy. When you print the Standard Contract and Consent Form, please print two copies. One will be for your records and one will be for mine.
